If you are living or working in one EU country but insured in another, you may be worried about how you will access healthcare. Form S1 is the document that solves this problem.
The S1 was previously known as the E106, E109, and E121. It entitles you to healthcare in an EU country that is not the one where you are insured. It is mainly useful for posted workers, cross-border workers, pensioners, and civil servants, along with their dependents. You request it in the country where you are insured, not the country where you plan to use it.
For anyone applying for residency in Spain as an EU citizen, this form matters even more directly: the S1 is a required document for that residency application.
